Format: Flighted by Handicaps. Please send a copy of each player’s lowest UDGA handicap or index or lowest 18-hole league handicap as of August1, 2010 with your entry form. Handicaps cannot be changed after deadline. Home golf course and phone # must be filled in. Entry forms without handicap info will be returned. If you do not have an established handicap, you must send 3 (18 hole) scorecards attested by golf course.
